Overview

Omega-3 fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) derived from fish oil, were evaluated in a randomized, double-blind, placebo-controlled clinical trial sponsored by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) for the treatment of IgA nephropathy in children and young adults. The trial, conducted by the Southwest Pediatric Nephrology Study Group and published by Hogg et al. in 2006, compared the efficacy of daily fish oil supplements against alternate-day prednisone in slowing renal function decline. These polyunsaturated fatty acids function as small molecule lipid mediators that modulate inflammatory pathways by competing with arachidonic acid for metabolism by cyclooxygenase (COX) and lipoxygenase (LOX) enzymes. This competition reduces the production of pro-inflammatory eicosanoids and cytokines, thereby potentially limiting mesangial immune-complex deposition and glomerular injury. In this research context, the intervention was used as a non-branded nutritional supplement rather than a regulated pharmaceutical drug.
