Overview

This is a **combination therapy** used primarily in cataract surgery. It consists of three components: - **Omidria**: An FDA-approved intraocular solution containing phenylephrine (1%) and ketorolac (0.3%) used in the irrigating solution during surgery. Its main actions are to maintain intraoperative mydriasis (pupil dilation), prevent miosis, and reduce postoperative ocular pain and inflammation by combining an alpha-adrenergic agonist (phenylephrine) and a nonsteroidal anti-inflammatory drug (ketorolac)[4][9]. - **Moxifloxacin**: A broad-spectrum fluoroquinolone antibiotic that acts by inhibiting bacterial DNA gyrase and topoisomerase IV, preventing bacterial replication and used to reduce the risk of postoperative endophthalmitis (infection)[1][2][3]. - **Triamcinolone acetonide**: A synthetic corticosteroid used intraocularly to control postoperative inflammation by suppressing multiple inflammatory pathways, thus reducing edema, pain, and the risk of cystoid macular edema[1][2][7]. The combination is often administered intraoperatively (Omidria in irrigation; moxifloxacin and triamcinolone as intraocular or transzonular injections or subconjunctival depot) to reduce the need for postoperative topical drop regimens, minimize compliance issues, and improve visual and inflammatory outcomes after cataract surgery[1][3][8]. This regimen is sometimes referred to as a “dropless” or “no-drop” approach in ophthalmic surgery.
