Overview

This intervention refers to a combination therapy regimen consisting of intramuscular (detrusor) injections of onabotulinumtoxinA (Botox) and standard oral anti-Parkinson's medications. Investigated by Zhujiang Hospital, this approach is specifically aimed at treating overactive bladder (OAB) symptoms in patients with Parkinson's disease. OnabotulinumtoxinA is a neurotoxin that cleaves SNAP-25, a protein essential for the docking and release of acetylcholine from presynaptic vesicles. By inhibiting acetylcholine release at the neuromuscular junction of the bladder detrusor muscle, it induces temporary muscle relaxation and reduces the urgency and frequency associated with OAB. The oral anti-Parkinson's component typically includes dopamine precursors (like levodopa) or dopamine agonists, which are used to manage the primary motor symptoms of the underlying neurodegenerative disease.
