OnabotulinumtoxinA is a purified form of botulinum toxin type A, a neurotoxic protein produced by Clostridium botulinum. It acts by inhibiting the release of acetylcholine at neuromuscular junctions, resulting in temporary muscle paralysis or relaxation. The combination "onabotulinumtoxina + hydrogel" refers to an investigational formulation where onabotulinumtoxinA is delivered via intravesical instillation using a hydrogel as the delivery vehicle rather than traditional intradetrusor injection. This approach aims to treat refractory overactive bladder with urgency urinary incontinence while reducing the invasiveness of administration. However, clinical trials have shown that this combination was well tolerated but did not demonstrate efficacy over placebo for this indication[4][5].
