OncoFAP Radio Conjugates (Therapy) is a radiopharmaceutical therapeutic program developed by Philogen S.p.A. targeting Fibroblast Activation Protein (FAP), which is highly expressed in the stroma of over 90% of epithelial cancers. The lead candidate, 177Lu-OncoFAP-23, is a small molecule radioconjugate consisting of a high-affinity FAP ligand, an innovative spacer, and a DOTAGA chelator labeled with the beta-emitting radioisotope Lutetium-177 (177Lu). This therapeutic approach is designed to deliver targeted irradiation to the tumor microenvironment, specifically the FAP-expressing stroma, across various solid tumors including breast, colorectal, lung, and pancreatic cancers. The program includes multiple variants such as the monomeric 177Lu-OncoFAP and the dimeric 177Lu-BiOncoFAP, with 177Lu-OncoFAP-23 being a multivalent (homotrimeric) conjugate that has demonstrated favorable biodistribution and potent anticancer activity in preclinical models.
