ONCOS-102 + cyclophosphamide + pemetrexed + cisplatin is an investigational combination therapy being studied primarily in malignant pleural mesothelioma and possibly other solid tumors. ONCOS-102 is a genetically modified oncolytic adenovirus (Ad5/3-D24-GMCSF) that selectively infects and lyses cancer cells with p16/Rb-defective phenotypes, while encoding granulocyte-macrophage colony-stimulating factor (GM-CSF) to stimulate anti-tumor immunity by enhancing dendritic cell and T-cell recruitment[1][2][3][5][6][7]. Cyclophosphamide is an alkylating agent that suppresses regulatory immune cells, pemetrexed is an antifolate inhibitor of DNA synthesis, and cisplatin is a platinum-based DNA crosslinker suppressing tumor growth. The combined regimen aims to remodel the tumor microenvironment, promote immune cell infiltration (notably CD4+ and CD8+ T cells), and improve overall survival beyond chemotherapy alone in treatment-naive mesothelioma patients. Evidence supports increased immune activation and improved tumor control when ONCOS-102 is added to pemetrexed and platinum chemotherapy[5]. This combination remains investigational and is not approved for any indication.
