**ONCOS-102 + pembrolizumab** is an investigational combination therapy for cancer, specifically for anti–PD-1–refractory advanced melanoma. It comprises two agents: - **ONCOS-102**, an oncolytic adenovirus engineered to express granulocyte-macrophage colony-stimulating factor (GM-CSF), designed to selectively infect and lyse tumor cells, trigger local inflammation, and recruit immune cells, thereby priming tumors for immune attack and enhancing immunogenicity. - **Pembrolizumab** is a monoclonal antibody checkpoint inhibitor targeting programmed cell death protein 1 (PD-1), blocking its interaction with ligands and thereby releasing inhibition of cytotoxic T cells. The combination aims to remodel the tumor microenvironment through ONCOS-102-induced inflammation and immune activation, increasing cytotoxic CD8+ T-cell infiltration and enabling renewed sensitivity to PD-1 blockade[2][3]. Early-phase clinical studies have shown that ONCOS-102 + pembrolizumab can yield objective responses in patients whose melanoma progressed on anti-PD-1 therapy, with an objective response rate of around 35%. Reduction in both injected and non-injected lesions indicates systemic immune activation[1][2][3].
