Ontorpacept + doxorubicin is a combination therapy evaluated primarily for advanced leiomyosarcoma and other soft tissue sarcomas. Ontorpacept (TTI-621) is a recombinant fusion protein that acts as a CD47-blocking agent, designed to enhance anti-tumor immune responses by inhibiting the "don't eat me" signal on cancer cells, thereby promoting phagocytosis by macrophages. Doxorubicin is an anthracycline chemotherapy agent that intercalates DNA, inhibits topoisomerase II, and prevents cancer cell replication. The rationale for the combination is that ontorpacept may enhance the effectiveness of doxorubicin by modulating the tumor microenvironment and stimulating immune-mediated tumor cell clearance, complementing the direct cytotoxic effect of doxorubicin. This drug combination was under development by Trillium Therapeutics (acquired by Pfizer) but associated clinical trials have been terminated for administrative reasons, not for safety concerns[1][3][5].
