Opioid-Free Anesthesia (OFA) is a specialized anesthetic protocol developed and studied by the Medical University of Lodz for morbidly obese patients undergoing laparoscopic bariatric surgery. The regimen is designed to provide complete surgical anesthesia and analgesia without the use of opioids (such as fentanyl), thereby avoiding common side effects like respiratory depression, sedation, and opioid-induced hyperalgesia, which are particularly risky in the obese population. The protocol utilizes a synergistic combination of ketamine, lidocaine, magnesium sulfate, dexamethasone, paracetamol, and rocuronium bromide. These agents work through various mechanisms, including NMDA receptor antagonism, sodium channel blockade, and glucocorticoid signaling, to maintain hemodynamic stability and ensure patient comfort during and after the procedure.
