oral corticosteroid + immunosuppressive agents

This treatment regimen represents the systemic therapy arm of the Multicenter Uveitis Steroid Treatment (MUST) Trial, a randomized Phase 4 comparative effectiveness study led by the JHSPH Center for Clinical Trials. The regimen consists of oral corticosteroids, primarily prednisone, as the first-line treatment for vision-threatening non-infectious intermediate uveitis, posterior uveitis, or panuveitis. Immunosuppressive agents (such as methotrexate, mycophenolate mofetil, or azathioprine) are added as needed for corticosteroid-sparing purposes or when inflammation is inadequately controlled. The goal of this approach is to achieve and maintain quiescent disease while minimizing the systemic side effects associated with long-term, high-dose corticosteroid use. In the MUST trial, this systemic approach was compared against the fluocinolone acetonide intraocular implant, demonstrating similar long-term visual outcomes with a different profile of adverse events.
