This combination therapy consists of oregovomab, a murine monoclonal antibody, and pegylated liposomal doxorubicin (PLD), a cytotoxic anthracycline. Oregovomab targets the tumor-associated antigen CA-125 (Mucin-16), which is highly expressed in various gynecological malignancies. Rather than simple neutralization, oregovomab acts as an indirect immunotherapeutic agent by forming immune complexes with circulating CA-125, which are then processed by antigen-presenting cells to induce a robust T-cell mediated immune response against the tumor. PLD provides a complementary cytotoxic effect by intercalating into DNA and inhibiting topoisomerase II, with its pegylated liposomal formulation allowing for prolonged circulation and enhanced accumulation in tumor tissues. This specific combination is being investigated in the Phase II OPERA trial (KGOG 3065/APGOT-OV6), led by Yonsei University, specifically for patients with ovarian, fallopian tube, or primary peritoneal cancers that have become resistant to PARP inhibitors or platinum-based chemotherapies.
