oritavancin + warfarin

Oritavancin is a semisynthetic lipoglycopeptide antibiotic indicated for the treatment of acute bacterial skin and skin structure infections (ABSSSI) caused by susceptible Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA). It acts through three mechanisms: inhibition of transglycosylation and transpeptidation steps in cell wall biosynthesis, and disruption of the bacterial cell membrane. Warfarin is an oral anticoagulant that acts as a vitamin K antagonist by inhibiting the vitamin K epoxide reductase complex subunit 1, thereby preventing the synthesis of active clotting factors II, VII, IX, and X. Oritavancin does not have a clinically meaningful effect on warfarin pharmacokinetics when co-administered; however, patients should be monitored for bleeding if both drugs are used together[2][3][5][6][8].
