Overview

This combination therapy regimen, primarily investigated by Memorial Sloan Kettering Cancer Center, consists of the third-generation EGFR tyrosine kinase inhibitor osimertinib paired with platinum-based chemotherapy (cisplatin or carboplatin) and etoposide. It is specifically designed for patients with metastatic EGFR-mutant non-small cell lung cancer (NSCLC) who also harbor concurrent TP53 and RB1 genetic alterations. These triple-mutant patients are at a significantly higher risk of undergoing histological transformation to small cell lung cancer (SCLC), a common mechanism of resistance to EGFR inhibitors. The regimen aims to eradicate pre-existing small cell clones or prevent transformation by combining targeted therapy with SCLC-directed cytotoxic chemotherapy.
