Overview

Otlertuzumab + ibrutinib is an investigational combination therapy consisting of a CD37-directed ADAPTIR protein and a Bruton's tyrosine kinase (BTK) inhibitor. Otlertuzumab (TRU-016) is a humanized, single-chain, homodimeric therapeutic protein that targets CD37, a tetraspanin protein highly expressed on B-cell malignancies. It induces cell death through direct caspase-independent apoptosis and antibody-dependent cell-mediated cytotoxicity (ADCC). Ibrutinib is a small molecule that covalently binds to BTK, inhibiting the B-cell receptor signaling pathway which is essential for the survival and proliferation of malignant B cells. This combination was evaluated by Aptevo Therapeutics in a Phase 1b clinical trial for patients with chronic lymphocytic leukemia (CLL), aiming to leverage the complementary mechanisms of action of both agents to improve therapeutic outcomes in B-cell malignancies.
