Overview

**OTX-TKI + anti-VEGF** is a combination therapy consisting of a **sustained-release intravitreal axitinib implant (OTX-TKI)** and an **anti-VEGF agent** (such as aflibercept or ranibizumab) used for ophthalmic indications, most notably neovascular (wet) age-related macular degeneration (nAMD). The OTX-TKI implant is a bioerodible, polyethylene glycol (PEG)-based hydrogel that delivers axitinib (a highly selective small molecule inhibitor of VEGF receptors 1, 2, 3 and PDGF receptors) directly into the vitreous, providing sustained intraocular concentrations for extended disease suppression. The combination leverages dual mechanisms of VEGF pathway blockade: the anti-VEGF agent sequesters extracellular VEGF ligands, while axitinib inhibits downstream intracellular signaling by blocking receptor tyrosine kinase activity. This approach aims to improve treatment durability, reduce injection frequency, and address both extracellular and intracellular VEGF-driven angiogenesis in retinal diseases.
