Ovarian function suppression + aromatase inhibitor is a combination therapy used primarily in premenopausal women with hormone receptor-positive (estrogen receptor-positive) breast cancer. Ovarian function suppression (OFS) can be achieved through medications such as gonadotropin-releasing hormone (GnRH) agonists (e.g., goserelin), surgery, or radiation to suppress estrogen production by the ovaries. Aromatase inhibitors (AIs), such as anastrozole or exemestane, block the conversion of androgens to estrogens in peripheral tissues. This combination is necessary in premenopausal women because AIs alone are ineffective when the ovaries are still producing estrogen; OFS reduces circulating estrogen to postmenopausal levels, allowing AIs to work effectively. Clinical trials have shown that this regimen improves disease-free survival and reduces recurrence risk compared to tamoxifen-based therapies in high-risk premenopausal patients[1][2][3][4].
