This is a combination chemotherapy regimen consisting of oxaliplatin, a platinum-based antineoplastic agent, and 5-fluorouracil (5-FU), a pyrimidine analog antimetabolite. In the context of research led by Peking University, this combination is administered via hepatic arterial infusion chemotherapy (HAIC) for the treatment of locally advanced perihilar cholangiocarcinoma (pCCA). Oxaliplatin works by forming DNA cross-links, which inhibit DNA replication and transcription, leading to cell cycle arrest and apoptosis. 5-fluorouracil acts as an antimetabolite that inhibits thymidylate synthase, thereby disrupting the synthesis of thymidine required for DNA repair and replication. The use of HAIC allows for the delivery of high concentrations of these cytotoxic agents directly into the hepatic artery supplying the tumor, aiming to maximize local efficacy while reducing systemic side effects in patients with advanced biliary tract cancers.
