oxaliplatin + capecitabine is a combination chemotherapy regimen, commonly referred to as XELOX or CAPOX, consisting of the platinum-based agent oxaliplatin and the oral fluoropyrimidine prodrug capecitabine. Oxaliplatin exerts its cytotoxic effect by forming DNA cross-links, which inhibit DNA replication and transcription, leading to cell cycle arrest and apoptosis. Capecitabine is enzymatically converted to 5-fluorouracil (5-FU) within the body, which then inhibits thymidylate synthase, an enzyme essential for DNA synthesis. This combination is a standard-of-care treatment for various gastrointestinal malignancies. In a clinical trial led by Peking Union Medical College Hospital, this regimen is being evaluated as a neoadjuvant therapy, both as a standalone control and in combination with the PD-1/VEGF bispecific antibody ivonescimab (AK112), for patients with locally advanced gastric adenocarcinoma or gastroesophageal junction (GEJ) adenocarcinoma that contains signet ring cells.
