Overview

This is a chemotherapy regimen administered via hepatic arterial infusion (HAI) for the treatment of advanced, unresectable biliary tract carcinomas. Developed and studied by researchers at Peking University Cancer Hospital & Institute, the protocol involves the regional delivery of oxaliplatin and 5-fluorouracil (5-FU) directly into the hepatic artery, often supplemented by intravenous leucovorin, followed by maintenance therapy with oral capecitabine. Oxaliplatin is a platinum-based antineoplastic agent that induces DNA cross-linking, thereby inhibiting DNA replication and transcription. Fluorouracil and its oral prodrug capecitabine are pyrimidine analogs that act as thymidylate synthase inhibitors, disrupting DNA synthesis. This localized delivery method aims to achieve high drug concentrations within the liver, where biliary tract tumors are primarily located, while minimizing systemic toxicity compared to standard intravenous chemotherapy.
