Oxaliplatin + irinotecan + folinic acid + fluorouracil (specifically the sequential FOLFOX7 followed by FOLFIRI regimen) is a combination chemotherapy strategy developed by the GERCOR (Groupe Coopérateur Multidisciplinaire en Oncologie) for the treatment of resectable metastatic colorectal cancer. This regimen was evaluated in the Phase III MIROX trial, which compared a sequential approach—6 cycles of FOLFOX7 (high-dose oxaliplatin, folinic acid, and 5-fluorouracil) followed by 6 cycles of FOLFIRI (irinotecan, folinic acid, and 5-fluorouracil)—against the standard FOLFOX4 regimen. The primary objective of this sequential strategy is to maintain high anti-tumor efficacy while reducing the cumulative neurotoxicity typically associated with prolonged oxaliplatin exposure. The components work through complementary mechanisms: oxaliplatin forms DNA adducts, irinotecan inhibits topoisomerase I, and fluorouracil (potentiated by folinic acid) inhibits thymidylate synthase, collectively disrupting DNA replication and repair in malignant cells.
