Overview

Oxaliplatin + raltitrexed + pirarubicin is a combination chemotherapy and embolization regimen developed by the Second Affiliated Hospital of Guangzhou Medical University for the treatment of unresectable large hepatocellular carcinoma (HCC). The protocol combines drug-eluting bead transarterial chemoembolization (DEB-TACE) using pirarubicin-loaded microspheres with hepatic artery infusion chemotherapy (HAIC) using the RALOX regimen (oxaliplatin and raltitrexed). Oxaliplatin acts as a DNA alkylating agent, raltitrexed as a thymidylate synthase inhibitor, and pirarubicin as a DNA intercalator and topoisomerase II inhibitor. This intensive local delivery system is designed to improve efficacy in large tumors (>7cm) while reducing the systemic toxicity and infusion time associated with traditional regimens like FOLFOX-HAIC.
