oxaliplatin + tegafur + gimeracil is a combination chemotherapy regimen primarily utilized in the treatment of advanced gastric and gastroesophageal junction cancers. This combination forms the therapeutic core of the SOX regimen, which is a standard of care in East Asia. Oxaliplatin is a platinum-based antineoplastic agent that works by forming inter- and intra-strand DNA cross-links, thereby inhibiting DNA replication and transcription. Tegafur is a prodrug of 5-fluorouracil (5-FU), an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. Gimeracil is a dihydropyrimidine dehydrogenase (DPD) inhibitor that prevents the degradation of 5-FU, significantly increasing its systemic exposure and antitumor efficacy. Clinical development and optimization of this regimen in China have been led by institutions such as Jinling Hospital, Medical School of Nanjing University, particularly through large-scale trials like the RESOLVE study.
