The P regimen (Prophase) is a cytoreductive chemotherapy combination protocol developed by the Children's Cancer Group, China (CCCG) for the initial treatment of pediatric and adolescent patients with newly diagnosed high-risk anaplastic large cell lymphoma (ALCL). It serves as a pre-phase to the modified ALCL99 regimen, aiming to rapidly reduce tumor burden and minimize the risk of tumor lysis syndrome before the administration of more intensive chemotherapy courses (Courses A and B). The regimen typically consists of dexamethasone and cyclophosphamide, administered over five days, along with intrathecal therapy for central nervous system prophylaxis. This specific protocol is currently being evaluated in a Phase 3 clinical trial to assess its efficacy when integrated into a modified ALCL99 backbone that includes vinorelbine.
