This neoadjuvant chemotherapy regimen, evaluated by Emory University, consists of paclitaxel (Taxol), cisplatin, and 5-fluorouracil (5-FU), administered alongside granulocyte colony-stimulating factor (G-CSF) support, typically filgrastim. Paclitaxel functions as a microtubule-stabilizing agent that inhibits mitosis, while cisplatin acts as a DNA-crosslinking platinum compound that induces apoptosis. 5-fluorouracil is an antimetabolite that targets thymidylate synthase to disrupt DNA synthesis. The addition of G-CSF is intended to mitigate the hematologic toxicity, specifically neutropenia, associated with this intensive triplet chemotherapy. This combination was investigated in a Phase II clinical trial for patients with resectable local-regional esophageal carcinoma to assess its efficacy in achieving pathological complete response and improving surgical outcomes.
