Paclitaxel + cisplatin + irinotecan is a combination chemotherapy regimen investigated by Memorial Sloan Kettering Cancer Center (MSKCC) for the treatment of advanced solid tumors, with a particular focus on gastrointestinal malignancies such as gastric and esophageal adenocarcinoma. The regimen combines three cytotoxic agents with distinct mechanisms: paclitaxel, a taxane that stabilizes microtubules to inhibit mitosis; cisplatin, a platinum-based agent that forms DNA cross-links to induce apoptosis; and irinotecan, a topoisomerase I inhibitor that prevents DNA strand religation. A Phase I dose-escalation study was conducted to determine the maximum tolerated dose and safety profile of this triplet combination, identifying neutropenia as the primary dose-limiting toxicity.
