Overview

This is a combination chemotherapy regimen developed and evaluated by Yonsei University for the treatment of gastric cancer with peritoneal metastasis. The regimen integrates multiple therapeutic modalities, including cytoreductive surgery (CRS), hyperthermic intraperitoneal chemotherapy (HIPEC) using cisplatin and paclitaxel, and postoperative intraperitoneal (IP) chemotherapy consisting of IP paclitaxel, IP cisplatin, and oral S-1 (a fluoropyrimidine combining tegafur, gimeracil, and oteracil). Paclitaxel acts as a microtubule stabilizer, cisplatin as a DNA cross-linking agent, and S-1 as a prodrug of 5-fluorouracil that inhibits thymidylate synthase. The treatment strategy aims to maximize local drug concentration in the peritoneal cavity while minimizing systemic toxicity, specifically targeting peritoneal carcinomatosis from gastric origin.
