Overview

This high-dose combination chemotherapy regimen, followed by autologous progenitor cell transplantation (ASCT), was investigated by the National Cancer Institute (NCI) for the treatment of stage IIIB inflammatory breast cancer (IBC). The protocol involves an induction phase with paclitaxel and cyclophosphamide, followed by a consolidation phase using high-dose melphalan and etoposide. Paclitaxel acts as a microtubule stabilizer, preventing the disassembly of tubulin and arresting the cell cycle. Cyclophosphamide and melphalan are nitrogen mustard alkylating agents that form DNA cross-links, leading to cell death. Etoposide is a topoisomerase II inhibitor that induces double-stranded DNA breaks. Because the high-dose consolidation phase is severely myelosuppressive, autologous progenitor cells are reinfused to rescue and restore the patient's hematopoietic system.
