This is a combination regimen consisting of **paclitaxel**, a microtubule-stabilizing chemotherapeutic; **HM30181A**, a potent and selective P-glycoprotein (P-gp/ABCB1) inhibitor that increases the central nervous system and systemic bioavailability of paclitaxel; and **pembrolizumab**, a monoclonal antibody directed against programmed death receptor-1 (PD-1) on T-cells, thereby unleashing an antitumor immune response. This combination is intended to enhance the delivery and efficacy of paclitaxel (which is limited by P-gp-mediated drug efflux, notably at the blood-brain barrier and in resistant tumor cells), potentially potentiated by the immune activation from pembrolizumab. It is under early-stage investigation for its ability to increase the anticancer effect in tumors where penetration and resistance mechanisms have limited the efficacy of paclitaxel-based chemotherapy, including possibly brain tumors and other solid tumors[2][4].
