Overview

paclitaxel + nab-paclitaxel refers to the use of taxane-based chemotherapy agents, either in conventional or nanoparticle albumin-bound (nab) formulations, within a specific clinical treatment context. In the Phase II LUNG-IST-127 trial sponsored by the University of Illinois at Chicago, these agents are utilized as part of an induction regimen alongside carboplatin and pembrolizumab for patients with metastatic squamous non-small cell lung cancer (sqNSCLC). Both paclitaxel and nab-paclitaxel function as microtubule stabilizers; they bind to the beta-subunit of tubulin, promoting the assembly of microtubules and inhibiting their disassembly. This stabilization interferes with the normal dynamic reorganization of the microtubule network required for mitosis, leading to cell cycle arrest at the G2/M phase and subsequent apoptosis. Nab-paclitaxel (Abraxane) is a solvent-free formulation that utilizes albumin nanoparticles to deliver paclitaxel, potentially enhancing tumor uptake via gp60-mediated transcytosis and reducing the risk of hypersensitivity reactions associated with the Cremophor EL vehicle used in standard paclitaxel (Taxol).
