Overview

This combination therapy involves the administration of intraperitoneal (IP) paclitaxel alongside the systemic NALIRIFOX regimen, which consists of liposomal irinotecan, 5-fluorouracil, leucovorin, and oxaliplatin. Developed and evaluated by the University of Colorado, Denver, this specific protocol is designed to treat pancreatic ductal adenocarcinoma (PDAC) that has metastasized to the peritoneum (peritoneal carcinomatosis). The rationale for the intraperitoneal administration of paclitaxel is to achieve high local concentrations of the cytotoxic agent within the abdominal cavity to directly target peritoneal surface malignancies while minimizing systemic side effects. Paclitaxel acts as a microtubule stabilizer to inhibit mitosis, while the NALIRIFOX components provide multi-targeted systemic effects through DNA alkylation, topoisomerase I inhibition, and antimetabolite activity.
