Overview

Paclitaxel + nedaplatin is a chemotherapy combination regimen that has been studied primarily for the treatment of esophageal squamous cell carcinoma (ESCC) and epithelial ovarian cancer (EOC). This combination pairs two established anticancer agents with complementary mechanisms of action. ## Composition and Mechanism The combination consists of paclitaxel, a taxoid chemotherapeutic agent that acts as a mitotic inhibitor, and nedaplatin, a platinum-based compound similar to cisplatin but with potentially different toxicity profiles. Paclitaxel works by stabilizing microtubules and preventing their breakdown during cell division, while nedaplatin forms DNA crosslinks that inhibit DNA replication and transcription, leading to cell death. ## Clinical Applications This regimen has shown promising results in several clinical settings: - **Esophageal Squamous Cell Carcinoma (ESCC)**: Phase II studies have demonstrated that paclitaxel + nedaplatin is active and well-tolerated as first-line therapy for patients with metastatic ESCC, with overall response rates of 41.7-46.1%. - **Epithelial Ovarian Cancer**: The combination has been compared with paclitaxel + carboplatin (the standard treatment) in a multi-center, randomized, open-label phase III trial, showing comparable survival outcomes with similar toxicity profiles. - **Advanced Urothelial Cancer**: A modified regimen including paclitaxel, ifosfamide, and nedaplatin (PIN) has been studied in patients with advanced or recurrent urothelial cancer who had received prior cisplatin-based therapy, showing an 83% overall response rate. ## Dosing Regimen The typical dosing schedule includes: - Paclitaxel: 175 mg/m² administered as a 3-hour intravenous infusion on day 1 - Nedaplatin: 80 mg/m² administered as a 1-hour intravenous infusion on day 2 (or day 1 in some protocols) - Treatment cycle repeated every 3 weeks ## Toxicity Profile Common adverse effects include: - Hematological toxicities: neutropenia (15.4% grade 3/4), anemia (7.7% grade 3/4), thrombocytopenia (5.1% grade 3/4) - Gastrointestinal toxicities: nausea (10.3% grade 3/4), vomiting (5.1% grade 3/4) - Febrile neutropenia (2.6% grade 3/4) The toxicity profile appears manageable, with most studies reporting no treatment-related deaths. A newer formulation using albumin-bound paclitaxel combined with nedaplatin has also been studied as neoadjuvant therapy for ESCC, showing a higher pathological complete response rate and fewer side effects compared to conventional formulations.
