Overview

This is a four-drug combination chemotherapy regimen consisting of paclitaxel, oxaliplatin, leucovorin (folinic acid), and 5-fluorouracil (5-FU). The regimen is commonly referred to as POFL or POF. It has been investigated primarily for the treatment of advanced or metastatic gastric cancer and pancreatic cancer. Paclitaxel is a microtubule-stabilizing agent that inhibits cell division; oxaliplatin is a platinum-based DNA crosslinker causing apoptosis; 5-fluorouracil is an antimetabolite that inhibits thymidylate synthase and disrupts DNA synthesis; leucovorin enhances the binding of 5-FU to its target enzyme, increasing cytotoxicity. This combination has demonstrated significant activity with manageable toxicity in clinical trials for recurrent/metastatic gastric cancer and advanced pancreatic cancer[1][2][3][4][7][9].
