Overview

This combination regimen is a multi-agent neoadjuvant chemo-immunotherapy consisting of the microtubule-stabilizing taxane paclitaxel, the PD-1 checkpoint inhibitor pembrolizumab, and the dual HER2-targeted monoclonal antibodies trastuzumab and pertuzumab. It is primarily investigated for the treatment of previously untreated, stage II-III, human epidermal growth factor receptor 2 (HER2)-positive breast cancer. The regimen aims to leverage the synergy between HER2-directed therapy and immune checkpoint inhibition to improve pathologic complete response (pCR) rates. Trastuzumab and pertuzumab provide dual HER2 blockade by binding to distinct epitopes of the receptor, while paclitaxel provides cytotoxic activity and pembrolizumab enhances the anti-tumor immune response by blocking the PD-1/PD-L1 inhibitory pathway.
