The combination of paclitaxel protein bound (nab-paclitaxel), gemcitabine, cisplatin, and paricalcitol is a neoadjuvant chemotherapy regimen being evaluated by the HonorHealth Research Institute for the treatment of pancreatic adenocarcinoma. This four-drug regimen, sometimes referred to as G-PaC plus paricalcitol, is specifically studied in patients with untreated resectable, borderline resectable, or locally advanced pancreatic ductal adenocarcinoma. The regimen's primary goal is to achieve CA 19-9 normalization prior to surgical resection. Nab-paclitaxel acts as a microtubule stabilizer to inhibit cell division, while gemcitabine and cisplatin serve as cytotoxic agents that interfere with DNA synthesis and repair. Paricalcitol, a vitamin D receptor (VDR) agonist, is included to modulate the dense tumor stroma characteristic of pancreatic cancer, potentially enhancing the delivery and efficacy of the cytotoxic agents by remodeling the tumor microenvironment.
