Palbociclib is a selective, oral small molecule inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), developed by Pfizer. It is used in combination with endocrine therapies as an initial or subsequent treatment for hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-) advanced or metastatic breast cancer. In the neoadjuvant setting, palbociclib is combined with various forms of endocrine therapy—such as aromatase inhibitors (letrozole, anastrozole) or selective estrogen receptor degraders/modulators—to block estrogen-driven tumor growth and cell cycle progression. Palbociclib works by inhibiting CDK4/6-mediated phosphorylation of the retinoblastoma protein, thereby arresting cells in the G1 phase and preventing proliferation of cancer cells[1][3][5][8]. Neoadjuvant endocrine therapy refers to hormone-blocking treatment given before surgery to shrink tumors in HR+ breast cancer.
