**Palonosetron + fosnetupitant** is a fixed-dose antiemetic combination drug marketed as **Akynzeo** for intravenous use. It contains **palonosetron**, a serotonin 5-hydroxytryptamine type 3 receptor antagonist that primarily prevents acute chemotherapy-induced nausea and vomiting, and **fosnetupitant**, a prodrug of netupitant that acts as a neurokinin 1 receptor antagonist to help prevent both acute and delayed chemotherapy-induced nausea and vomiting. The combination is administered as a single intravenous dose before chemotherapy, typically with dexamethasone, and is used in adults to prevent nausea and vomiting associated with initial and repeat courses of highly emetogenic cancer chemotherapy. Both active ingredients are small molecules; fosnetupitant is converted in vivo to netupitant, the pharmacologically active NK1 receptor antagonist.
