Overview

This is a combination of three bisphosphonate drugs—pamidronate, risedronate, and zoledronic acid. Each is a nitrogen-containing bisphosphonate that inhibits osteoclast-mediated bone resorption, thereby reducing bone turnover and increasing bone mineral density. These agents are primarily used to treat conditions such as osteoporosis, Paget’s disease of bone, hypercalcemia of malignancy, multiple myeloma with bone involvement, and metastatic bone disease from solid tumors. The mechanism involves binding to hydroxyapatite in the bone matrix and inhibiting farnesyl pyrophosphate synthase in osteoclasts, leading to reduced osteoclast activity and apoptosis[1][2][5][6]. While each drug is approved for use individually or sometimes sequentially with other therapies (such as alendronate), there is insufficient evidence or clinical precedent for the routine use of this specific triple combination in standard practice[7].
