This is a combination regimen consisting of four drugs: - **Panitumumab** is a fully human monoclonal antibody targeting the epidermal growth factor receptor (EGFR), inhibiting its signaling and thereby blocking tumor cell proliferation and survival. - **Epirubicin** is an anthracycline chemotherapy agent that intercalates DNA, inhibits topoisomerase II, and generates free radicals, leading to cytotoxicity in rapidly dividing cells. - **Oxaliplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks, inhibiting DNA replication and transcription. - **Capecitabine** is an oral prodrug converted to 5-fluorouracil (5-FU) in the body; it inhibits thymidylate synthase, disrupting DNA synthesis. This combination was investigated as first-line therapy for advanced or metastatic oesophagogastric adenocarcinoma. The REAL3 phase 3 trial evaluated this regimen but found no improvement in overall survival with the addition of panitumumab to epirubicin, oxaliplatin, and capecitabine compared to chemotherapy alone. The study was halted early due to inferior outcomes with the combination including panitumumab[1][2][4].
