Overview

The panobinostat + gemcitabine + busulfan + melphalan (Pano/Gem/Bu/Mel) regimen is a high-dose conditioning therapy evaluated in Phase 2 clinical trials at the M.D. Anderson Cancer Center. It is designed for patients with relapsed or refractory lymphomas, including diffuse large B-cell lymphoma, Hodgkin lymphoma, and T-cell lymphoma, who are undergoing autologous stem cell transplantation (ASCT). The regimen combines panobinostat, a pan-histone deacetylase (HDAC) inhibitor, with the nucleoside analog gemcitabine and the DNA alkylating agents busulfan and melphalan. Preclinical studies suggest that panobinostat synergistically enhances the cytotoxic effects of these chemotherapy agents by modulating chromatin structure and inducing apoptosis. In some cases, rituximab is added to the regimen for patients with CD20-positive tumors.
