This is a combination regimen consisting of pantoprazole (a proton pump inhibitor), amoxicillin (a beta-lactam antibiotic), and levofloxacin (a fluoroquinolone antibiotic). The combination is primarily used as a triple therapy for the eradication of Helicobacter pylori infection, particularly in cases where first-line regimens have failed or are unsuitable due to resistance or intolerance. Pantoprazole suppresses gastric acid secretion by inhibiting the H+/K+ ATPase enzyme in gastric parietal cells, creating an environment less favorable for H. pylori survival and enhancing antibiotic efficacy. Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis. Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication and repair, resulting in bactericidal activity against susceptible organisms including H. pylori.
