Parecoxib + dexmedetomidine is a combination therapy consisting of a selective cyclooxygenase-2 (COX-2) inhibitor and a selective alpha-2 adrenergic agonist. Parecoxib, a water-soluble prodrug of valdecoxib, provides potent anti-inflammatory and analgesic effects by inhibiting prostaglandin synthesis. Dexmedetomidine is a highly selective alpha-2 adrenergic agonist that provides sedation, analgesia, and sympatholytic effects without significant respiratory depression. This combination is primarily being investigated by the Second Affiliated Hospital of Jiaxing University for its potential to reduce the incidence and severity of postoperative cognitive dysfunction (POCD), particularly in elderly patients undergoing major surgeries such as laparoscopic or orthopedic procedures. The rationale for the combination is that parecoxib mitigates the systemic and neuroinflammatory responses to surgical stress, while dexmedetomidine provides neuroprotection and reduces the requirement for other anesthetics that may exacerbate cognitive decline.
