Overview

The combination of pazopanib, gemcitabine, and docetaxel (GDP) has been studied as a treatment regimen for soft tissue sarcomas (STS), particularly in the neoadjuvant setting. This combination brings together gemcitabine and docetaxel, which are frequently used together for STS, with pazopanib, a multityrosine kinase inhibitor that has shown activity in STS. A phase Ib/II trial investigated this triple combination (GDP) in patients with high-grade extremity soft tissue sarcomas larger than 8 cm who were candidates for definitive resection. The regimen consisted of: - Gemcitabine 900 mg/m² on days 1 and 8 - Docetaxel 75 mg/m² on day 8 - Pazopanib 400 mg daily This trial was discontinued due to slow accrual after enrolling only five patients. The results showed no complete or partial responses, although one patient achieved ≥90% pathologic response. Three patients discontinued treatment due to significant toxicity, which included grade 3 hypertension, fatigue, liver enzyme elevation (AST/ALT), hoarseness, and myelotoxicity. The incidence of hepatotoxicity was particularly concerning. Concomitant treatment with pazopanib has been reported to increase docetaxel exposure, potentially explaining the increased toxicity. While this triple combination showed significant toxicity, other related combinations have been studied, such as Gemcitabine + Pazopanib and Gemcitabine + Docetaxel, showing varying efficacy and toxicity profiles.
