PCI-27483 + gemcitabine is a combination investigational regimen consisting of **PCI-27483**, a selective small molecule inhibitor of activated factor VII (factor VIIa), and **gemcitabine**, a nucleoside analog antimetabolite that blocks DNA synthesis. PCI-27483 inhibits the tissue factor (TF)/factor VIIa (FVIIa) complex, thereby preventing activation of downstream protease-activated receptor 2 (PAR-2) signaling, which is implicated in tumor growth, angiogenesis, metastasis, and the promotion of venous thromboembolism in some cancers. Gemcitabine interferes with DNA synthesis, leading to antitumor activity. This combination was studied primarily in advanced pancreatic cancer, where it was hypothesized to reduce thromboembolic complications and exert antineoplastic effects. In Phase 2 studies, the combination was well tolerated but did not demonstrate significant efficacy improvement compared to gemcitabine alone[1][2][3][4].
