PCV7 + PPV23 is a combined pneumococcal vaccination strategy that uses two different types of pneumococcal vaccines in sequence. This approach aims to provide broader protection against pneumococcal disease by leveraging the benefits of both vaccine types. ## Description PCV7 + PPV23 is a sequential vaccination strategy that combines the 7-valent pneumococcal conjugate vaccine (PCV7) followed by the 23-valent pneumococcal polysaccharide vaccine (PPV23). This combination approach was developed to maximize protection against pneumococcal disease. PCV7 (Prevnar) contains purified capsular polysaccharides from 7 serotypes of Streptococcus pneumoniae (4, 6B, 9V, 14, 18C, 19F, and 23F) individually conjugated to a non-toxic variant of diphtheria toxin called CRM197 protein[8][10]. The conjugation process enhances immune response by recruiting T-cell help, which allows for immunoglobulin type switching and production of memory B cells[9]. This results in more robust immunity, particularly in children. PPV23 (Pneumovax 23) contains purified polysaccharide antigens from 23 types of pneumococcal bacteria without protein conjugation[1][7]. It works primarily by stimulating B-cells to release IgM without T-cell assistance[9]. The rationale behind this combined approach is that PCV7 induces a stronger initial immune response and immunological memory, while the subsequent PPV23 broadens protection to include additional serotypes. However, studies in certain populations, such as adult transplant recipients, have questioned whether there is significant additional benefit from the PPV23 dose after PCV7 administration[6]. This combination strategy has been largely superseded by newer pneumococcal conjugate vaccines with broader serotype coverage (PCV13, PCV15, PCV20, and PCV21), which are now recommended in many vaccination schedules[3][4].
