PD-L1 antibody + bevacizumab + HAIC-FOLFOX is a combination chemo-immunotherapy regimen being evaluated for the treatment of advanced unresectable hepatocellular carcinoma (HCC) with extrahepatic metastases. The regimen consists of a PD-L1-targeting monoclonal antibody (dosed at 1200mg, which is characteristic of atezolizumab or sugemalimab), the anti-angiogenic monoclonal antibody bevacizumab, and Hepatic Arterial Infusion Chemotherapy (HAIC) using the FOLFOX protocol (oxaliplatin, leucovorin, and 5-fluorouracil). This triple-modality approach, investigated by West China Hospital, aims to combine the systemic effects of immune checkpoint blockade and anti-angiogenesis with the localized, high-concentration cytotoxic effects of HAIC. By delivering chemotherapy directly to the liver via the hepatic artery, the regimen seeks to enhance local tumor control while the systemic components address extrahepatic spread, potentially improving outcomes for patients with high tumor burden and limited treatment options.
