Overview

This entry refers to the combination of a programmed death-ligand 1 (PD-L1) monoclonal antibody and a cytotoxic T lymphocyte-associated protein 4 (CTLA-4) monoclonal antibody. Both are immune checkpoint inhibitors used in cancer immunotherapy. PD-L1 antibodies block the interaction between PD-L1 on tumor cells and PD-1 on T-cells, thereby enhancing T-cell-mediated immune responses against tumors. CTLA-4 antibodies inhibit the CTLA-4 receptor on T-cells, further promoting immune activation by preventing downregulation of T-cell activity. The rationale for combining these two classes is to achieve synergistic enhancement of antitumor immunity by targeting different regulatory pathways in the immune response[2][5]. This combination is under investigation for several cancers including hepatocellular carcinoma (HCC), non-small cell lung cancer (NSCLC), bladder cancer, and others[2][5]. Examples of specific drugs used in such combinations include durvalumab (a PD-L1 inhibitor) with tremelimumab (a CTLA-4 inhibitor)[2].
