Overview

This is a combination therapy consisting of a PD-L1 monoclonal antibody (such as durvalumab or atezolizumab) and lenalidomide. The PD-L1 monoclonal antibody is an immune checkpoint inhibitor that blocks the interaction between programmed death-ligand 1 (PD-L1) on tumor cells and programmed death-1 (PD-1) receptors on T cells, thereby enhancing anti-tumor immune responses. Lenalidomide is an immunomodulatory drug (IMiD) that exerts direct anti-tumor effects, modulates the tumor microenvironment, enhances T cell and natural killer cell activity, downregulates regulatory T cells, and reduces expression of both PD-1 on effector cells and PD-L1 on tumor cells. The combination aims to synergistically boost immune-mediated cytotoxicity against cancer by simultaneously blocking inhibitory signals in the immune system while stimulating effector cell function. This regimen has been investigated in various hematologic malignancies including multiple myeloma, cutaneous T-cell lymphoma (CTCL), diffuse large B-cell lymphoma (DLBCL), and acute myeloid leukemia[4][6][7][8].
