Pectuna + trastuzumab is a combination therapy consisting of Pectuna, a pertuzumab biosimilar developed by the Iranian biotechnology company CinnaGen, and the monoclonal antibody trastuzumab. Pectuna is a recombinant humanized monoclonal antibody that targets the extracellular dimerization domain (Subdomain II) of the human epidermal growth factor receptor 2 (HER2). By binding to this specific domain, it sterically hinders the dimerization of HER2 with other HER family members, such as HER1 (EGFR), HER3, and HER4, thereby blocking downstream signaling pathways like PI3K/AKT and MAPK that drive tumor cell proliferation and survival. Trastuzumab binds to a different epitope on HER2 (Subdomain IV), providing complementary inhibition of ligand-independent signaling and mediating antibody-dependent cellular cytotoxicity (ADCC). This dual HER2 blockade is primarily used for the neoadjuvant treatment of HER2-positive breast cancer, typically administered in combination with chemotherapy regimens such as TCHP (docetaxel, carboplatin, trastuzumab, and pertuzumab) to maximize pathological complete response (pCR) rates.
