DDGP is a multi-agent chemotherapy regimen consisting of dexamethasone, cisplatin, gemcitabine, and pegaspargase. It was developed by researchers at Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, specifically for the treatment of extranodal natural killer/T-cell lymphoma (ENKTL). The regimen is designed to overcome the multidrug resistance often seen in NK/T-cell malignancies, which frequently express P-glycoprotein. Pegaspargase depletes asparagine, an essential amino acid for lymphoma cells; gemcitabine and cisplatin act as potent DNA-damaging agents; and dexamethasone provides both lymphocytotoxic effects and anti-emetic support. Clinical studies, including a landmark Phase 3 trial, have demonstrated that DDGP significantly improves progression-free survival and overall survival compared to the SMILE regimen in patients with newly diagnosed stage III/IV or relapsed/refractory ENKTL, while exhibiting a more manageable toxicity profile.
