Peginterferon + ribavirin + telaprevir is a triple combination antiviral regimen formerly used as the standard of care for the treatment of **chronic hepatitis C virus (HCV) genotype 1 infection**. Peginterferon (pegylated interferon alfa) is an immune modulator that stimulates the host antiviral response; ribavirin is a nucleoside analog with broad antiviral activity; and telaprevir is a small-molecule, oral **NS3/4A serine protease inhibitor** that directly inhibits HCV replication. Combined therapy resulted in higher sustained virological response (SVR) rates but was associated with significant adverse effects, especially cutaneous and hematologic toxicities. Telaprevir, developed by Vertex Pharmaceuticals and marketed as Incivek (also Incivo), was approved in 2011 but withdrawn from the market in 2014 due to the advent of better-tolerated and more effective HCV direct-acting antivirals[1][2][3][4][5][7].
